Permit & Notifications
- Is a demolition permit obtained with an approved demolition plan? *
- Has the required 10-working-day NESHAP demolition/renovation notification been submitted? *
- Are utility disconnect confirmations (gas, electric, water, sewer) documented? *
- Are adjacent property protection and pedestrian protection measures planned? *
Asbestos Survey (ACM)
- Has a pre-demolition asbestos survey been performed by a licensed/accredited inspector? *
- Are suspect materials sampled and analyzed (PLM), or assumed as ACM? *
- Is regulated asbestos-containing material (RACM) abated before demolition? *
- Are abatement waste manifests and disposal records retained? *
Lead & Other Regulated Materials
- Has a lead-based paint assessment been completed? *
- Are PCB-containing ballasts/transformers, mercury (lamps/thermostats), and refrigerants identified for removal? *
- Are universal wastes segregated and managed per RCRA? *
- Are underground/aboveground storage tanks identified and addressed? *
Demolition Safety
- Has an engineering survey of structural stability been performed before demolition? *
- Are dust control / water suppression measures in place? *
- Are exclusion zones and fall/void protection established? *
- Is worker respiratory and PPE protection provided per exposure assessment? *
Records
- Are survey reports, notifications, and disposal manifests compiled for the file?
- Photo of site pre-demolition condition
- Responsible party signature
