Design-Phase Documentation
- Is the Owner's Project Requirements (OPR) document complete and current? *
- Does the Basis of Design (BOD) address each OPR criterion? *
- Is a commissioning plan in place identifying systems, roles, and schedule? *
- Have Cx requirements been incorporated into the specifications and submittals? *
Commissioned Systems Scope
- Are HVAC, controls/BAS, and mechanical systems included in the commissioning scope? *
- Are lighting controls and daylighting systems commissioned? *
- Are domestic hot water and applicable renewable/energy systems included? *
- Are life-safety interfaces (smoke control, fire/HVAC) coordinated for testing? *
Prefunctional Verification
- Are prefunctional checklists completed for installed equipment prior to functional testing? *
- Is start-up documentation from manufacturers collected and reviewed? *
- Is testing, adjusting, and balancing (TAB) completed and reviewed? *
- Are control sequences and points lists verified against the BOD? *
Functional Performance Testing
- Are functional performance tests executed for each commissioned system? *
- Are equipment responses verified across normal, emergency, and failure modes? *
- Are integrated systems tests (e.g., emergency power, sequences) performed? *
- Are deficiencies logged in an issues log and tracked to resolution?
Turnover & Documentation
- Is a final commissioning report issued to the owner? *
- Is a systems manual / preliminary O&M documentation delivered? *
- Has owner/operator training been provided and documented? *
- Commissioning authority signature
