Aseptic Gowning Qualification Checklist

Qualifies personnel to gown and behave aseptically for entry into Grade A/B cleanroom environments, including visual, microbial, and behavioral assessment. Use for initial qualification and periodic requalification.

Training Prerequisites

  • Has the individual completed aseptic technique and gowning training before qualification? *
  • Is the person free of conditions (e.g., illness, exposed lesions) that increase contamination risk? *
  • Are approved gowning SOPs and the correct garment sizes available? *
  • Record the operator name/ID and qualification date.

Gowning Technique Assessment

  • Is hand hygiene/sanitization performed correctly at each required step? *
  • Is the gowning sequence performed in the correct order without touching outer surfaces to non-sterile items? *
  • Are all skin and hair fully covered by sterile garments (hood, mask, goggles, gown, gloves, boots)? *
  • Is the qualified assessor's observation of technique documented? *

Microbial Evaluation

  • Are contact plates/samples taken from defined gown and glove locations after gowning? *
  • Do gloved fingertip counts meet the applicable limits? *
  • Do gown location results meet the applicable microbial limits? *
  • Record the number of consecutive successful gowning sessions required for qualification.

Aseptic Behavior in the Cleanroom

  • Does the operator maintain slow, deliberate movements and avoid disrupting unidirectional airflow? *
  • Does the operator avoid reaching over exposed sterile product/components? *
  • Are interventions performed per approved aseptic technique? *

Qualification Decision and Maintenance

  • Is a qualification decision documented based on visual, microbial, and behavioral results? *
  • Is periodic requalification (typically annual, or per media fill participation) scheduled? *
  • Is the operator's qualification status tracked and linked to cleanroom access? *
  • Assessor sign-off on gowning qualification. *
