LASA List Management
- Does the facility maintain a current list of look-alike/sound-alike medications used at the organization? *
- Is the LASA list reviewed and updated at least annually against the ISMP list of confused drug names? *
- Are actions taken to prevent errors for the drugs on the facility list documented? *
Labeling & Storage Controls
- Is tall man (mixed-case) lettering applied to LASA drug names on shelf labels, screens, and MARs? *
- Are LASA medications physically separated or flagged with auxiliary warnings in storage? *
- Are automated dispensing cabinet alerts configured for confirmed LASA pairs?
Ordering & Verification
- Are indications included on orders for LASA medications to aid verification?
- Is order-entry configured to avoid dangerous abbreviations and auto-populate errors for LASA drugs? *
- Are staff educated on high-risk LASA pairs relevant to their unit? *
Monitoring & Reporting
- Are LASA-related errors and near misses reported and analyzed? *
- Are new medications screened for LASA potential before formulary addition? *
- Is LASA program effectiveness reviewed by the medication safety committee at least annually? *
